  • No. & date of certificate if an adolescent - This subsection of the Tamil Nadu Form 12 Register of Adult Workers has two parts - No. of certificate & date and Token No. giving reference to the certificates. The Manager has to fetch the details from the adolescent worker’s birth certificate or any document displaying the birth date
  • Remarks - In the final subsection of the Tamil Nadu Form 12 Register of Adult Workers, the Manager may enter any remarks about the concerned worker

  • Registration No. - Here, the Manager must enter the registration number of the factory as mentioned in the Factories Act, 1948 (Central Act LXIII of 1948)

  • Name of the worker - Enter the workers’ names in these boxes
  • Worker Identity No. - Enter the worker number as entered in the Photo Identity Card as mandated in Rule 103-C of the Tamil Nadu Factories Rules 1950

  • Date of entry into service - Enter the date on which the concerned worker joined the factory
  • Designation / Nature of Work - In this subsection of the Tamil Nadu Form 12 Register of Adult Workers
  • EPF No./ UAN No. - Enter the Employee Provident Fund or Universal Account Number of the worker
  • ESIC No. - Enter the worker’s ESIC (Employees’ State Insurance Scheme)
  • Date on which completion of 480 days of service - Enter the date on which the concerned worker has completed or will complete 480 days in service
  • Date on which made permanent - Enter the date on which the factory made the concerned worker a permanent staff
  • Period of Suspension if any - Enter the from and to date of such suspension
  • Bank A/c Number, Name of Bank, Branch (IFSC) - Enter the worker’s bank account number, bank name, branch name, and IFSC (Indian Financial System Code)
  • Photo - Attach the worker’s photograph
  • Mobile Number - Enter the worker’s mobile number
  • E-mail ID - Enter the worker’s email address
  • Specimen Signature / Thumb Impression - The Manager needs to ask the worker to sign in the appropriate box
  • Date of Exit - If the worker has left the factory, mention the date on which the worker left
  • Reason for Exit - Mention the reason(s) why the worker(s) left the factory
  • Remarks - The Manager may enter any remarks in this subsection of the Tamil Nadu Form 12 Register of Adult Workers
