What is Form 32 Registration?

The employers of all factories coming under the purview of the Factories Act 1948 and Manipur Factories Rules 2016 must maintain Form 32 Register of Accidents and Dangerous Occurrences or Form 32 Registration in the provided form.

The Manipur Factories Rules 2016, of which Form 32 Registration is a part, applies to all factories where more than ten (10) workers work using power or more than twenty (20) workers work without using power.

In Form 32 Registration, the owner or occupier of all registered factories operating in Manipur must declare the accidents that took place in the factory premises.

Which Compliance Forms Does a Manager Need to Prepare?

According to the Factories Act 1948 and the Manipur Factories Rules 2016, all managers, owners, or occupiers of factories registered and/or operating in the Indian State of Manipur must prepare and submit the following forms, besides Form 32 Registration or Form 32 Register of Accidents and Dangerous Occurrences:

How to Fill Form 32 - Register of Accidents and Dangerous Occurrences?

Name of Injured person

In this subsection of Form 32 Registration, the owner, occupier, or manager of a factory needs to mention the name(s) of the injured person(s) in a month.

The names mentioned here must be among the names mentioned in Form No. 31 - Muster Roll, which contains the name, father’s name, nature of work, and dates of attendance of all employees in a factory registered and/or operating in the Indian State of Manipur.

Hence, before preparing the Form 32 Registration, the manager, owner, or occupier of a factory must prepare the Muster Roll in Form 31 (Rule 152).

Date of accident or dangerous occurrence

In this subsection of Form 32 Registration, the owner, occupier, or manager of a factory needs to mention the date(s) in which the accident or dangerous occurrence took place.

According to Rule No. 144 (1) of the Manipur Factories Rules 2016, the owner, occupier, or manager of a factory must send a notice to the Chief Inspector and Inspector by a special messenger or telephone where an employee has received any bodily injury that may cause his/her death.

Rule 144 (1) further states that if any dangerous occurrence as listed on Schedule X takes place in the factory premises, the manager must inform the Chief Inspector and Inspector as well.

Moreover, according to Rule No. 144 (2) of the Manipur Factories Rules 2016, a copy of the report must be sent to the Sub-Divisional Officer (SDO) or the District Magistrate (DM), the local police station’s Officer-In-Charge, and the injured or deceased person’s relatives.

Date of report (in Form 25) to Inspector

In this subsection of Form 32 Registration, the owner, occupier, or manager of a factory needs to mention the date(s) in which the accident or dangerous occurrence was reported to the Chief Inspector and Inspector.

Nature of accident or dangerous occurrence

In this subsection of Form 32 Registration, the owner, occupier, or manager of a factory needs to mention the nature of the accident in which an employee received any bodily injury or dangerous occurrence as mentioned in Schedule X and prescribed under Rule 144.

A dangerous occurrence may refer to a plant burst, crane failure or overturning, and any molten metal’s leakage, bursting out, fire, or explosion. It may also refer to a container or receiver explosion or any gallery, floor tunnel, chimney, building, wall, or roof collapse.

Date of return of injured person to work

In this subsection of Form 32 Registration, the owner, occupier, or manager of a factory needs to mention the date on which an employee received a bodily injury or a victim of any dangerous occurrence as mentioned in Schedule X and prescribed under Rule 144 returns to work.

Number of days the injured person was absent from work

In this subsection of Form 32 Registration, the owner, occupier, or manager of a factory needs to mention the total number of days the concerned employee received a bodily injury or a victim of any dangerous occurrence as mentioned in Schedule X and prescribed under Rule 144 was absent from work.
